Portable equipment available to service patients on site at the facility at the patients convenience.  This saves times and costs for transportation of residents from your facility.

Possessing state-of-the-art equipment not yet available elsewhere. 

Repairs done to glasses at no extra charge (unless glasses parts needed).  We stock all frame styles used and can make emergency lens replacement same day in many cases

Adjustment done to glasses at no charge ( even if the patient does not require an examination).

New glasses dispensed on site at your nursing home.

Comprehensive eye examinations including dilated examination for pathology, prescription check, basic neurological examination, and tonometry---A report of findings will be placed in the patient's chart for use by staff and other professionals

Intermediate and follow-up exams such as glaucoma monitoring, cataract, diabetes, dry eyes, and macular degeneration evaluations

Emergency visits (i.e. infections, trauma, sudden loss of vision) available between our usual visits as needed.

Treatment of eye disease with pharmaceuticals ( i.e. infection, trauma, allergic conjunctivitis, uveitis, foreign body removal, angle-closure glaucoma).

Newest techniques and pharmaceuticals for treating glaucoma and dry eyes.

Referral to appreciate provider of tertiary care as coordinated with the patient's attending physician.

24 hour paging service in case of emergency assistance.

We are fully staffed and do not require an assistant provided by the home for examination of the patients.

Mobile Vision Unit for bringing refractor on site, allows more accurate prescriptions.  We are one of the nine practices in the United State with this equipment.

The only Optometrist in Ohio with the GDX Laser Nerve Fiber Layer Analyzer (for Glaucoma) and the Computerized Tear Microasssay(for Red Eye Diagnosis).

Free GDX screenings and Dry eye treatment for employees and families; reduced cost on employee and family eye exams and glasses.

 

Our Request of Your Facility:

A list of patients to be seen two days prior to our visit ( last minute changes to be made if necessary)
A consent form signed by the patient/guardian and the attending physician allowing us to provide services to the patient ( see attached copy)
One room (approximately 13' x 13' in size) for us to set up our equipment and examine the patient
A copy of each patient's admission cover sheet giving valid insurance information, date of birth, P.O.A., etc.
A copy of each patient's current medications and medical history (diagnosis)
